1.  Duly completed application form
2.  Valid passport or driving license for identification 
3.  Valid visa / proof for right to work in the UK
4.  Current CV
5.  Last three years employment history
6.  At least two professional references
7.  Degree qualifications certificate
8.  Proof of professional registration NMC/HCPC
9.  Enhanced DBS certificate 
10.  Proof of National Insurance number
11.  2 original proof of address dated with in the last three months (utility bill/bank statement)
12.  2 passport sized photo in plain background 
13.  Your local GP details
14.  Proof of immunisation (Hepatitis B, TB, Varicella,  Rubella and Measles). All this documents must be signed, stamped and verified by GP or occupational heath department
